Background: Serum interleukin 6 (IL-6) levels have been studied in the diagnostic evaluation of patients with severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) disease (COVID-19). Methods: We studied the utility of treatment with tocilizumab in COVID-19 patients (n=19) with a negative nasopharyngeal swab real time reverse transcriptase polymerase chain reaction (RT-PCR) test for SARS-CoV-2 who had suggestive computed tomography (CT) findings, namely, COVID-19 Reporting and Data System (CO-RADS) 4,5. Results: Receiver operator characteristic (ROC) curve analysis showed that serum IL 6 at a cut-off of >56.9 pg/L was a predictor of mortality in nasopharyngeal swab RT-PCR negative patients with suggestive CT findings. Tocilizumab had no significant effect on the mortality. Conclusions: In nasopharyngeal swab RT-PCR negative patients with suggestive chest CT findings, elevated serum IL-6 levels > 56.9 pg/L predicted mortality. However, treatment with tocilizumab had no effect on mortality.

The Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) causes Covid-19, an infectious illness. A methodology was created to track the vaccination history of people with the Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) that causes Covid-19, an infectious illness. The system operates on a Raspberry Pi processor that is designed to authenticate the vaccination records of individuals. The Vaccination Identification System consists of various components connected to the Raspberry Pi Zero 2W microprocessor, Pi camera, an LCD display, LED indicators, a buzzer, a DC servo motor, and a PCB converter. The proposed system grants access to vaccinated individuals and denies access to those who are not vaccinated.

Mobile computing is one of the significant opportunities that can be used for various practical applications in numerous fields in real life. Due to inherent characteristics of ubiquitous computing, devices can gather numerous types of data that led to innovative applications in many fields with a unique emerging prototype known as Crowd sensing. Here, the involvement of people is one of the important features and their mobility provides an exclusive opportunity to collect and transmit the data over a substantial geographical area. Thus, we put forward novel idea about Quality of Information (QOI) with unique parameters with opportunistic uniqueness of people's mobility in terms of sensing and transmission. Additionally, we propose some of the viable improved ideas about the competent opportunistic data collection through efficient techniques. This work also considered some of the open issues mentioned by previous related works.

RESUMO

The World Health Organization (WHO) has suggested a successful social distancing strategy for reducing the COVID-19 virus spread in public places. All governments and national health bodies have mandated a 2-m physical distance between malls, schools, and congested areas. The existing algorithms proposed and developed for object detection are Simple Online and Real-time Tracking (SORT) and Convolutional Neural Networks (CNN). The YOLOv3 algorithm is used because YOLOv3 is an efficient and powerful real-time object detection algorithm in comparison with several other object detection algorithms. Video surveillance cameras are being used to implement this system. A model will be trained against the most comprehensive datasets, such as the COCO datasets, for this purpose. As a result, high-risk zones, or areas where virus spread is most likely, are identified. This may support authorities in enhancing the setup of a public space according to the precautionary measures to reduce hazardous zones. The developed framework is a comprehensive and precise solution for object detection that can be used in a variety of fields such as autonomous vehicles and human action recognition. © 2023, The Author(s), under exclusive license to Springer Nature Singapore Pte Ltd.

The immune system protects the body from infectious agents such as bacteria, viruses, or fungi. Once encountered with pathogens or antigens, the innate and adaptive arms of the immune system trigger a strong immune response to eliminate them from the system and protect the body. Thus, well-balanced immunity is pivotal for maintaining human health, as an insufficient level of immune defense leads to infections and tumors. In contrast, the excessive functioning of the immune system causes the development of autoimmune diseases and allergies. Strong immunity requires adequate nutrition, dietary interventions, and sufficient intake of certain vitamins (vitamin C, vitamin D, and folic acid) and minerals (magnesium, zinc, and selenium). Therefore, nutritional and micronutrient deficiencies lead to compromised immunity. Several natural ingredients have shown potent immunomodulatory properties. The immune-enhancing properties of many plants and fungi are due to containing bioactive phytoconstituents such as polyphenols, terpenoids, ß-glucans, vitamins, etc. Probiotics and prebiotics can be used as innovative tools to reduce intestinal inflammation and downregulate hypersensitivity reactions. Plant sources of melatonin, a multifunctional molecule with proven anti-inflammatory and immunomodulatory properties, have been discovered relatively recently. The bioactive compounds augment the immune response by directly increasing the cytotoxic activity of natural killer cells, macrophages, and neutrophils. Many phytoconstituents prevent cell damage due to their powerful antimicrobial, antioxidant, and anti-inflammatory properties. The present review attempts to understand the molecular mechanisms underlying the immune-enhancing properties of some bioactive compounds from plants, fungi, animals, microorganisms, and other natural sources.

The coronavirus disease 2019 (COVID-19) pandemic still has tremendous impacts on the global socio-economy and quality of living. The traditional Chinese Medicines (TCM) approach showed encouraging results during previous outbreaks of Severe Acute Respiratory Syndrome-related coronavirus (SARS-CoV) and the Middle East Respiratory Syndrome Coronavirus (MERS-CoV). With limited treatment availability, TCM herbs and formulations could be viable to reduce COVID-19 symptoms and potential sources for discovering novel therapeutic targets. We reviewed 12 TCM herbs and formulations recommended for COVID-19 management by the National Health Commission and National Administration of Traditional Chinese Medicine, the People's Republic of China. This article explored the Chinese national authorities' guidelines from 2003 to 2020, the scientific data in public databases for the recommended TCM remedies, and their potential mechanistic actions in COVID-19 management. Several TCM herbs and formulations could potentially benefit COVID-19 management. The recommended TCM oral preparations list are Huoxiang zhengqi, Jinhua Qinggan, Lianhua Qingwen, and Shufeng jiedu; the recommended injection preparations comprise Xiyanping Xuebijing, Re-Du-Ning, Tanreqing, Xingnaojing, Shenfu, Shengmai, and Shenmai. TCM remedies are viable options for symptom alleviation and management of COVID-19. The current SARS-CoV-2 pandemic presents an opportunity to find novel therapeutic targets from TCM-active ingredients. Despite the recommendations in Chinese National guidelines, these remedies warrant further assessments in well-designed clinical trials for their efficacies in COVID-19.

Importance: Multisystem inflammatory syndrome in children (MIS-C) is a severe and unrestrained inflammatory response with multiorgan involvement, which occurs within a few weeks following the resolution of acute SARS-CoV-2 infection. The complement system is a vital part of the innate immune system and plays a role in COVID-19 pathogenesis. Objective: To examine and compare the levels of complement components and regulators along with complement activation products in the different clinical spectrum of children with SARS-CoV-2 and a control group. Design, Setting, and Participants: This cross-sectional study analyzed children with MIS-C admitted to a single hospital in India from June through September 2020. Eligible participants were children who were hospitalized of either sex, aged 1 to 18 years. Data were analyzed August 2022. Measures: Levels of complement components and regulators along with complement activation products in all the groups of children. Mann-Whitney U test and Kruskal-Wallis analysis were used to compare the complement component levels, and Spearman rank correlation analysis was used to describe the association between complement components and laboratory and biochemical parameters. Results: A total 145 children were included (median age, 5 years [range, 1 month-17 years); 84 [58%] male): 44 children with MIS-C, 33 with acute COVID-19 (reverse transcriptase-polymerase chain reaction [RT-PCR] positive), 47 with convalescent COVID-19 (immunoglobulin G-positive non-MIS-C) and 21 children for a control group (both serology and RT-PCR negative). Children with MIS-C and COVID-19 had higher levels of C1q (geometric mean [SD]: MIS-C, 61.5 [18.5] ng/mL; acute COVID-19, 56.9 [18.6] ng/mL; controls, 24.1 [3.3] ng/mL), C2 (MIS-C, 605.8 [219.7] ng/mL; acute COVID-19, 606.4 [167.7] ng/mL; controls, 255.9 [73.3] ng/mL), C3 (MIS-C, 318.2 [70.7] ng/mL; acute COVID-19, 237.7 [61.8] ng/mL; controls, 123.4 [15.7] ng/mL), C4b (MIS-C, 712.4 ng/mL; acute COVID-19, 640.7 ng/mL; controls, 351.5 ng/mL), C5 (MIS-C, 1487 ng/mL; acute COVID-19, 1364 ng/mL; controls, 561.9 ng/mL), C5a, (MIS-C, 2614.0 [336.2] ng/mL; acute COVID-19, 1826.0 [541.0] ng/mL; controls, 462.5 [132.4] ng/mL), C3b/iC3b (MIS-C, 3971.0 [635.1] ng/mL; acute COVID-19, 3702.0 [653.9] ng/mL; controls, 2039.0 [344.5] ng/mL), and factor B (MIS-C, 47.6 [7.8] ng/mL; acute COVID-19, 44.6 [6.3] ng/mL; controls, 27.5 [5.0] ng/mL), factor D (MIS-C, 44.0 [17.2] ng/mL; acute COVID-19, 33.8 [18.4] ng/mL; controls, 21.3 [6.1] ng/mL), and factor H (MIS-C, 53.1 [4.0] ng/mL; acute COVID-19, 50.8 [5.7] ng/mL; controls, 43.6 [3.8] ng/mL) in comparison with convalescent and control children. In addition, children with MIS-C had significantly elevated levels of C3 (318.2 [70.7] ng/mL vs 237.7 [61.8] ng/mL), C5a (2614 [336.2] ng/mL vs 1826 [541.0] ng/mL), and mannose-binding lectin (79.4 [12.4] ng/mL vs 69.6 [14.7] ng/mL) in comparison to children with acute COVID-19. Levels of some of these analytes at admission (ie, pretreatment) were more elevated in children with MIS-C who needed pediatric intensive care unit (PICU) support as compared with those who did not require PICU support, and in children with COVID-19 who developed moderate to severe disease compared with those who developed mild disease. Overall, MIS-C and acute COVID-19 were associated with the hyperactivation of complement components and complement regulators. Conclusions and Relevance: In this cross-sectional study, the complement system was associated with the pathogenesis of MIS-C and COVID-19 in children; complement inhibition could be further explored as a potential treatment option.

mRNA vaccines have been demonstrated as a powerful alternative to traditional conventional vaccines because of their high potency, safety and efficacy, capacity for rapid clinical development, and potential for rapid, low-cost manufacturing. These vaccines have progressed from being a mere curiosity to emerging as COVID-19 pandemic vaccine front-runners. The advancements in the field of nanotechnology for developing delivery vehicles for mRNA vaccines are highly significant. In this review we have summarized each and every aspect of the mRNA vaccine. The article describes the mRNA structure, its pharmacological function of immunity induction, lipid nanoparticles (LNPs), and the upstream, downstream, and formulation process of mRNA vaccine manufacturing. Additionally, mRNA vaccines in clinical trials are also described. A deep dive into the future perspectives of mRNA vaccines, such as its freeze-drying, delivery systems, and LNPs targeting antigen-presenting cells and dendritic cells, are also summarized.

Introduction: Multisystem Inflammatory Syndrome in children (MIS-C) is a serious inflammatory sequela of SARS-CoV2 infection. The pathogenesis of MIS-C is vague and matrix metalloproteinases (MMPs) may have an important role. Matrix metalloproteinases (MMPs) are known drivers of lung pathology in many diseases. Methods: To elucidate the role of MMPs in pathogenesis of pediatric COVID-19, we examined their plasma levels in MIS-C and acute COVID-19 children and compared them to convalescent COVID-19 and children with other common tropical diseases (with overlapping clinical manifestations). Results: Children with MIS-C had elevated levels of MMPs (P < 0.005 statistically significant) in comparison to acute COVID-19, other tropical diseases (Dengue fever, typhoid fever, and scrub typhus fever) and convalescent COVID-19 children. PCA and ROC analysis (sensitivity 84-100% and specificity 80-100%) showed that MMP-8, 12, 13 could help distinguish MIS-C from acute COVID-19 and other tropical diseases with high sensitivity and specificity. Among MIS-C children, elevated levels of MMPs were seen in children requiring intensive care unit admission as compared to children not needing intensive care. Similar findings were noted when children with severe/moderate COVID-19 were compared to children with mild COVID-19. Finally, MMP levels exhibited significant correlation with laboratory parameters, including lymphocyte counts, CRP, D-dimer, Ferritin and Sodium levels. Discussion: Our findings suggest that MMPs play a pivotal role in the pathogenesis of MIS-C and COVID-19 in children and may help distinguish MIS-C from other conditions with overlapping clinical presentation.

Background: A new type of Corona virus that is SARS - Cov2 called COVID 19 had a huge pandemic worldwide. On January 30,2020 the World health organization (WHO) declared the outbreak of covid19 as a public health emergency of international concern1 Methods: Descriptive and retrospective study carried out at general hospital, Sapthagiri institute of medical science and research centre from September 2020 to September 2021 Results: Among 153 tested neonates, 91 were sars-cov2 positive. Out of 91 (59%), most common symptom reported is respiratory distress in the form of TTNB(43%) and require respiratory support for longer period compared to covid negative group. Conclusion: 55% of neonates were symptomatic and reported higher incidence of NICU admission rates in SARS-COV-2 positive neonates born to SARS-COV-2 infected mothers which is comparable to our study. The clinical presentation of MIS-C overlaps with other infectious/non-infectious diseases such as acute COVID-19, Kawasaki disease, acute dengue, enteric fever, and systemic lupus erythematosus. We examined the ex-vivo cellular parameters with the aim of distinguishing MIS-C from other syndromes with overlapping clinical presentations. MIS-C children differed from children with non-MIS-C conditions by having increased numbers of naïve CD8+ T cells, naïve, immature and atypical memory B cells and diminished numbers of transitional memory, stem cell memory, central and effector memory CD4+ and CD8+ T cells, classical, activated memory B and plasma cells and monocyte (intermediate and non-classical) and dendritic cell (plasmacytoid and myeloid) subsets. All of the above alterations were significantly reversed at 6-9 months post-recovery in MIS-C. Thus, MIS-C is characterized by a distinct cellular signature that distinguishes it from other syndromes with overlapping clinical presentations. RESUMO

COVID-19 pandemic has become the most devastating disease of the current century and spread over 216 countries around the world. The disease is spreading through outbreaks despite the availability of modern sophisticated medical treatment. Machine Learning and Image Analysis research has been making great progress in many directions in the healthcare field for providing support to subsequent medical diagnosis. In this paper, we have propose three research directions with methodologies in the fight against the pandemic namely: Chest X-Ray (CXR) images classification using deep convolution neural networks with transfer learning to assist diagnosis; Patient Risk prediction of pandemic based on risk factors such as patient characteristics, comorbidities, initial symptoms, vital signs for prognosis of disease; and forecasting of disease spread & case fatality rate using deep neural networks. Further, some of the challenges, open datasets and opportunities are discussed for researchers.

T cells are thought to be an important correlates of protection against SARS-CoV2 infection. However, the composition of T cell subsets in convalescent individuals of SARS-CoV2 infection has not been well studied. The authors determined the lymphocyte absolute counts, the frequency of memory T cell subsets, and the plasma levels of common γ-chain in 7 groups of COVID-19 individuals, based on days since RT-PCR confirmation of SARS-CoV-2 infection. The data show that both absolute counts and frequencies of lymphocytes as well as, the frequencies of CD4+ central and effector memory cells increased, and the frequencies of CD4+ naïve T cells, transitional memory, stem cell memory T cells, and regulatory cells decreased from Days 15-30 to Days 61-90 and plateaued thereafter. In addition, the frequencies of CD8+ central memory, effector, and terminal effector memory T cells increased, and the frequencies of CD8+ naïve cells, transitional memory, and stem cell memory T cells decreased from Days 15-30 to Days 61-90 and plateaued thereafter. The plasma levels of IL-2, IL-7, IL-15, and IL-21-common γc cytokines started decreasing from Days 15-30 till Days 151-180. Severe COVID-19 patients exhibit decreased levels of lymphocyte counts and frequencies, higher frequencies of naïve cells, regulatory T cells, lower frequencies of central memory, effector memory, and stem cell memory, and elevated plasma levels of IL-2, IL-7, IL-15, and IL-21. Finally, there was a significant correlation between memory T cell subsets and common γc cytokines. Thus, the study provides evidence of alterations in lymphocyte counts, memory T cell subset frequencies, and common γ-chain cytokines in convalescent COVID-19 individuals.

Throughout the pre-monsoon, monsoon, and post-monsoon seasons, the participants in this research took water samples that were later examined for a variety of physiochemical characteristics. Irrigation water quality metrics such as SAR, RSC, Na%, KR, MHR, PS, PI, and RSBC were used in conjunction with the calculation of IWQI in order to conduct an evaluation of the appropriateness of the irrigation water. The major dominated anions in the study area were found HCO3−> SO42−> Cl− during all the season, while cation was fluctuated from Na+> Mg2+> Ca2+> K+ at pre-monsoon season and Na+> Ca2+> Mg2+> K+ at both monsoon and post-monsoon season. Ca (Mg)- HCO3−, Na- HCO3−and mixed type water was found in the study area which was originated from the silicate weathering and evaporate dissolution. Some samples were reported with high KR, MHR, Na%, RSC, PS and RSBC which exceeding limit and unsuitable for irrigation use. Out of the total collected water samples 32%, 64%, 8%, 44%, 12%in pre-monsoon and 24%, 60%, 8% 16%, 12% in post-monsoon period were not suitable for irrigation purpose with having high KR, MHR, Na%, RSC and PS, respectively. While, 8% (KR), 52% (MHR), 12%(RSC) and 8% (PS) was also unsuitable for irrigation purpose during monsoon period. However, the IWQI of the study area was noted 48% water sample having above the limit > 100 which was unsuitable for irrigation water.

In the absence of a vaccine, timely and accurate COVID-19 coronavirus identification is essential for avoiding and managing the pandemic through immediate quarantine and medical treatment. The growing number of COVID-19 patients around the world, along with the limited number of available testing kits, makes detecting the condition difficult.Using current deep learning models, we analyze X-ray pictures and classify them as positive or negative for COVID-19 in this work (VGG19 and VGG16). The suggested method contains a preprocessing stage with data augmentation, which removes irrelevant information from the environment that could lead to biased findings following that, the classification model is trained using a transfer learning strategy using various location X-rays. © 2022 IEEE.

Aim and background: Coronavirus disease 2019 (COVID-19) pandemic caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has a high incidence of patients with the severe acute respiratory syndrome (SARS). Many of these patients require admission to an intensive care unit (ICU) for invasive ventilation and are at very high risk of developing ventilator-associated pneumonia (VAP).1-3 Objectives: To study the incidence and mortality of VAP of ventilated COVID-19 patients. Materials and methods: We retrospectively collected data on all patients hospitalized for COVID-19 during the first phase of the epidemic in a 9 bed ICU who were on invasive mechanical ventilation for >48 h. We studied the characteristics of VAP in these patients. VAP was diagnosed based on official recommendations. Results: Data of 55 patients was analyzed. Of these 23 (41.8%) had VAP. Patients with VAP required a greater number of ventilatory days (13.6 ± 6.2 vs 6 ± 4) and underwent more tracheostomies [9 (39.13%) vs 1 (3.13%)]. Length of ICU and hospital stay were significantly prolonged in COVID-19 VAP group 17.9 ± 11 vs 9.18 ± 6.11 days (p = 0.0002) and 18.3 ± 11 vs 9.9 ± 6.4 days (p = 0.0004), respectively. Also, organ involvement was significantly higher in the VAP group. Patients in both group had similar mortality, VAP - 15 (65.2%) vs non VAP - 18 (56.25%). Conclusion: COVID-19 is associated with an increased risk of VAP, which is not fully explained by the prolonged duration of ventilation. Complications like organ involvement and prolonged ventilatory days are common in COVID-19 VAP patients and need to focus on general supportive treatment and organspecific treatment.

The COVID-19 outbreak seems to be the most dangerous challenge of the third millennium due to its highly contagious nature. Amongst natural molecules for COVID-19 treatment, the flavonoid molecule quercetin (QR) is currently considered one of the most promising. QR is an active agent against SARS and MERS due to its antimicrobial, antiviral, anti-inflammatory, antioxidant, and some other beneficial effects. QR may hold therapeutic potential against SARS-CoV-2 due to its inhibitory effects on several stages of the viral life cycle. In fact, QR inhibits viral entry, absorption, and penetration in the SARS-CoV virus, which might be at least partly explained by the ability of QR and its derivatives to inhibit 3-chymotrypsin-like protease (3CLpro) and papain-like protease (PLpro). QR is a potent immunomodulatory molecule due to its direct modulatory effects on several immune cells, cytokines, and other immune molecules. QR-based nanopreparations possess enhanced bioavailability and solubility in water. In this review, we discuss the prospects for the application of QR as a preventive and treatment agent for COVID-19. Given the multifactorial beneficial action of QR, it can be considered a very valid drug as a preventative, mitigating, and therapeutic agent of COVID-19 infection, especially in synergism with zinc, vitamins C, D, and E, and other polyphenols.

RESUMO

The current study was conducted with sampling of eighty-four groundwater sample which was used for analyzing various physico- chemical properties in groundwater to assess the status of irrigation water quality and their hydro-geochemistry.For this purpose, different ionic concentration pH, EC, TDS, TH, Ca2+, Mg2+, Na+, K+, Cl−, SO42−, CO32− and HCO3− were determine in groundwater and used to calculate the various irrigation water quality parameters like SAR, Na %, RSC, MHR, KR, PI%, PS and RSBC. The groundwater sample pH was ranged from 7.9 to 9.3 with having electrical conductivity of 90 µS/cm to 2000 µS/cm. Among the all-sampled groundwater 2.38%, 1.19%, 5.95%, 16.67%, and 2.38% water samples were unsuitable for irrigation water due to having high loading of PI, KR, MHR, RSC and PS value, respectively. The ionic concentration orders werefound in terms of cations Ca2+> Na+> Mg2+> K+ and anions HCO3−> Cl−> SO42−in groundwater. The piper trilinear diagram suggested Ca (Mg)- HCO3−, Ca (Mg)-Cl− and mixed type water was dominated in the study area. The Gibbs and other bivariate plot were indicated that the silicate weathering and ion exchange phenomenafrom rock water interaction was mainly responsible for the different ion’s sources in groundwater instead of evaporite and carbonate dissolutions.

RESUMO

The prefatory protein-glycan interaction and stabilizing protein-protein interaction of severe acute respiratory syndrome viruses with angiotensin-converting enzyme 2 play a significant role in complex formation thereby promoting endocytosis. The microevolution of SARS-CoV-2 over a period of time has a significant role in increasing the affinity of receptor-binding domain against angiotensin converting-enzyme 2. In the current study, we have corroborated the vitality of acquired SNPs over a period of time with increased affinity by using docking studies. The results indicate that the virus modulates the undesirable glycosylation sites by a series of substitution and deletion mutations. It uses bulky residues such as Tyr/Phe for dynamic arrest for quick stabilization of the complex, and Lys residues for stabilizing via hydrogen bond formation besides increasing the binding affinity to ease the cell entry.
